An All Saints’ Church Directory will be available in late June!

No one’s information will be published without explicit consent. Therefore, for you to be included in the Directory, you must fill out a Directory Information Form.

You can complete a Directory Information Form online here or by downloading this Word document and typing in your information.

Or, you can complete a form by hand, either by printing out the Word document OR by picking up a form from the Information Table in the Atrium.

We’re aware that we didn’t make provision for spouses’ phone numbers and email addresses. Please add that information to your form, or each spouse can complete their own individual form and we’ll take care of putting it all together.

You are not required to provide every piece of information the form requests. Only the information you submit will be included in the Directory.

By submitting a Directory Information Form, you consent to the information you provide being made available in a hard-copy, printed format to the All Saints’ Church congregation.

Completed forms must be submitted by May 31 for inclusion in this year’s Directory.
